Principles of our barrier-based incident investigation tool

DNV GL has been helping clients for many years with incident investigations using its root cause analysis tool SCAT. In that time DNV GL has expanded on SCAT to create an incident investigation methodology focusing on barriers. This methodology allows incidents to be investigated regardless of size or complexity, to delve through immediate and basic causes to expose the underlying management system failures.

This webinar will discuss about the principles of our barrier-based incident investigation tool and explain how this can be used to investigate incidents and to ensure you learn the most from unwanted events.

Watch this webinar to learn:

  • Why we should focus on barriers for incident investigation?
  • How to identify barriers as part of an incident investigation?
  • Common challenges faced in incident investigations.
  • Examples of practical approaches to applying BSCAT methodology.
